Yes fill it out, sign it, and scan it. If you are under 18 you will need a parent signature.

Might I ask who this artist is? And will the winners be given any credit, such as a plug for winning? Or is there any form of a prize? Is there some sort of theme to follow or is it just random?

So, this seems awesome, but kind of vague. I mean, as a writer, I have half a dozen characters just sitting on a shelf in my mind waiting to be created, but what are you looking for? Is there a theme? A preference? A guideline? How about restrictions? Do you want them human? Or can they be demon, angel, elf, fairy, ghost, mecha, etc? Anything to narrow it down a bit?

You will receive a plug in the con guide and have your chibi plastered everywhere.
